SIG COVID19 BR session debates the pediatric emergency challenges in the pandemic

At the session on 09/28, SIG COVID19 BR of the University Tele-medicine Network (RUTE) promoted a debate about the pediatric emergency challenges in the pandemic, with the presence of Prof. Dr. Andréa Fraga, emergency pediatrician from the Pediatrics Department of FCM-Unicamp, and moderated by the group coordinators Prof. Dr. Evelyn Eisenstein and Dr. Luiz Ary Messina.

In the introduction of her lecture, Dr. Andrea showed what has been done at the emergency of the Unicamp hospital to prepare to receive the children during the new coronavirus pandemic, such as, for example, installation of external tents to receive and screen the suspected covid patients and training with the medical team and staff. The whole structuring was based on the guidelines from the World Health Organization (WHO) and the Brazilian Pediatric Society (SBP).

“We have always esteemed emergency service. We became more focused in this systematized assessment, in order not to miss any serious suspected case. We wanted to identify early in order to propose isolation, try to diagnose as soon as possible, and obviously, intervene”, the pediatrician told.

Then, the physician approached the Pediatric Multisystem Inflammatory Syndrome, which surprised the health area around the world.  While everyone thought that the patients suspected of being infected would be the ones with respiratory conditions, this didn´t happen. There were recurring cases of children and adolescents with abdominal ache, diarrhea, vomiting, fever, skin rash and conjunctivitis, as consequent symptoms of Covid-19.

The overview led to association with the Kawasaki disease and investigations started, leading to the finding of MIS, where there is genetic predisposition, causality relation to Sars-COV-2 and apparently, it is about an immune-mediated disease, in which the virus is the trigger. Until August, 197 cases were notified in Brazil.

“Treatment is multifunctional. Several professionals together bringing their expertise to manage treating a patient with this condition. Attention shall be paid to the quick clinical evolution and aggravation of the condition. It is not a long-term disease. The cases evolve quickly, so we shall pay attention to deterioration, when assisting these patients. Therefore, the best place for patients with this syndrome is intensive therapy”, Dr. Andréa pointed out.
